You may use any provider in the nation to obtain full benefits under your plan.  

  • PHYSICIAN and ANCILLARY CLAIMS in the PHCS network are paid at the in-network level. To locate a physician in the PHCS network, CLICK HERE (then Select Network > PHCS > Practitioner & Ancillary).
  • Physician claims outside of the PHCS network are paid at a multiple of the Medicare allowance for the service.  You are never responsible for more than the copay, coinsurance and/or deductible under your plan. 

  • FACILITY CLAIMS (hospitals etc) are paid at a multiple of the Medicare allowance for the service. The vast majority of providers will accept this amount. In fact, your plan guarantees that they will.  You are never responsible for more than the copay, coinsurance and/or deductible under your plan. 

If you incorrectly receive a "balance bill" from your provider that exceeds the amount you are responsible for on your EOB (Explanation of Benefits), contact Allied National's Elite Experience Team for assistance.

Most lab charges are covered under your plan and may be subject to your deductible and coinsurance. If you incur covered lab charges through LabCorp or Quest Diagnostics, they are covered at 100% with no deductible, therefore, tell your physician to use those labs, if possible. If your physician uses a different outside lab provider, visit your local LabCorp or Quest lab office to get 100% payment of your charges.
